That is why Cornerstone is here; to provide the finest Hospice and Palliative (comfort) care to our patients and their families. Our interdisciplinary team of healthcare professionals takes the time to learn what is important to each patient and uses this input to develop a personalize plan of care. Cornerstone Hospice is a not-for-profit community hospice which provides care to anyone facing a life-limiting illness, regardless of their ability to pay.
Community: There is a Need to Keep Hospice Patients Close to Family and Friends
With all of the wonderful amenities West Orange County offers its residents, there is not a designated in-patient hospice facility to serve this community. That is why we are opening the Cornerstone Hospice Care Center at Health Central. The West Orange Healthcare District has generously committed $1.25 million of the $2.6 million in building costs to construct this 10 bed facility inside the new skilled care and rehabilitation center on the Health Central campus This beautiful new building will allow those needing in-patient care to remain in their hometown community while still providing access to the highest quality services available..
While most patients prefer to remain at home, there are times when homecare may no longer be possible. This unique healthcare option allows patients to receive more intense symptom management while experiencing the home-like setting and amenities the Care Center has to offer. Keeping patients close to their home allows family and friends to spend more time by their loved one’s side as guests are welcome anytime, day or night.
Collaboration: There is a Need to Develop Innovative Community Partnerships
That is why Cornerstone Hospice and Orlando Health have partnered to bring our patients and their families the broadest healthcare options available. Working together to seamlessly transfer a patient from curative care to comfort care enables Cornerstone Hospice to compassionately guide the patient and family through this most difficult time. This reduces stress and anxiety and allows families to focus on what is most important.
