Hospice care and services provided in the home is the primary form of care—traditional home, nursing home, assisted living facility or any place considered to be the patient’s home.
Cornerstone’s focus is to keep patients with life-limiting illnesses comfortably at home for as long as possible. A specially trained team of professionals and caregivers deliver care for the “whole person” including physical, emotional, social and spiritual needs. Following an initial assessment of patient and family needs, all services that may benefit the patient will be coordinated with the primary caregiver. Care includes:
- Medical Supplies and Equipment related to hospice diagnosis
- Consultations by physical, occupational, speech and dietary therapists
- Related outpatient procedures and lab work
Each patient’s needs are continuously assessed and all treatment options are explored and evaluated within the patient’s values and belief system.
