About Us

A Cornerstone of the Community. Since 1984.

Since 1984, we have made it our mission to provide quality care to those affected by life-limiting illnesses. As a community-based, not-for-profit provider of hospice and palliative care services, we’re committed to caring for and improving the lives of our patients, their families, and their caretakers.

Our Mission

The Mission of Cornerstone Hospice and Palliative Care is to make quality Hospice care available to all persons, their families, and those affected by serious illness, death and dying in our community; to advocate effectively for patients’ comfort, dignity and choice; and to be recognized as the leading resource in clinical, ethical, and spiritual issues of dying and grief.

Our Values

A PROMISE MADE TO OUR PATIENTS: With compassion and a holistic approach, Cornerstone Hospice and Palliative Care employees, volunteers and contracted staff will render the highest quality of care and support:

  • Preserving hope, dignity, confidentiality and patient autonomy
  • Respecting individuality, personal choice, cultural diversity and individual spiritual beliefs
  • Recognizing and supporting the strengths of patients and their significant others
  • Delivering services regardless of ability to pay
  • Assuring excellence in symptom control and psychological and spiritual support
  • Respecting and preserving relationships with attending physicians

Our History

Cornerstone was founded out of a need, not an opportunity, in 1981 by a compassionate group of nurses in Eustis who were called to help aging neighbors in the community. Their dream became reality in the mid 80s when Lake Hospice was licensed to provide comprehensive hospice services to the community.

In February 1991, Tavares Hospice House with six beds opened next door to the Center, becoming the second hospice house built in the State and the first free-standing one. The organization began operating as Cornerstone Hospice & Palliative Care in 2010 to better represent the seven counties served.

Today, nearly 40 years later, we have an even greater reach and more diversified solutions that bring comfort and peace to everyone we serve, not only in one of our facilities, but even in the comfort of home.

Cornerstone Hospice services are available in Central Florida throughout all the cities and towns located in Lake, Sumter, Orange, Osceola, Polk, Hardee and Highlands counties, including but not limited to Leesburg, Mount Dora, Clermont, The Villages, Tavares, Lady Lake, Sumterville, Bushnell, Orlando, Winter Park, Kissimmee, Celebration, Lakeland, Auburndale, Bartow, Bay Lake, Haines City, Sebring, and Wauchula.

With the Cornerstone Hospice and Palliative Care mission, vision and values, along with the Hospice philosophy of care as the guiding principles, our care providers deliver the most comprehensive end-of-life care to patients and their families when the patient has a serious, complex, or advanced chronic disease or injury by providing symptom management and relief of pain — whether physical, emotional, or spiritual.

Cornerstone is consistently recognized as the leading resource in clinical, ethical, and spiritual issues of dying and grief, and we were recently awarded a prestigious accreditation from The Joint Commission for excellence in patient and family satisfaction. Other licenses and affiliations include:

  • Joint Commission Accredited
  • National Partnership for Hospice Innovation
  • Florida Hospices and Palliative Care Association
  • Medicare and Medicaid Certified
  • Florida State License #5019096, Agency for Health Care Administration

As our story continues to evolve through the decades, we remain committed to caring for and improving the lives of our residents and neighbors under the community-based, not-for-profit provider of hospice and palliative care services designation we started from.
